Everything was fine then GPU miner stopped accepting.
ATI 5870
10.8.5
MM 1.5.18e
Flags: -I 8 -w 64
LTC: stratum+tcp://usa3.coinhuntr.com:3334 (in legacy window)
Mine set to Scrypt

When Intensity was set to 11 there where a lot of HW errors.

How long has it been running there? Looks as though it just started and that's why no shares processed yet?

It had been running for hours when I noticed there where no accepted shares.
It closes my pool and moves to another where I don't have an account.
It has many HW errors if I let it run.
Used to be no HW errors.
Its had 3 HW errors while I'm typing this.
Zero accepted, zero rejected. Now 4 HW errors.

If that's the case please run the new pool setup which should be more reliable, if it's a problem getting stuck on a backup pool that should fix it

I've tried that.
The HW error is:

[2014-02-27 04:24:54] OCL 0: invalid nonce (2ee51100) - HW error

These keep occuring.

I ran bfgminer in the command line and there where no HW errors.
Jobs where accepted.
Speed was slow but I didn't mess with the settings.
The CPU did not run though (I used the d 0 flag)

Are you Litecoin mining? What happens if you turn the intensity down, does it work again?

After clicking all sorts of options in the
setup screen it finally started to work.
I did select recommended values and then
tweaked the options.
Perhaps that reset something.
May I suggest a set all to defaults/reset option.

Stopped again.
I have no idea what I did last time to get it working.

have you set extra options in the config file? There's nothing in what you just posted that looks wrong, is it just returning hardware errors again?

Now its working again at intensity 9 and worksize 64 and 10MH/s
but the monitor is jerky/delayed. Everything else is on automatic.

Now at intensity 11 and worksize 64
the monitor is unusable and the hash rate ~25MH/s.
With these same settings (intensity 11 and worksize 64)
before I was getting 110MH/s and the monitor was fine.

Very strange.

I'd be worried there's something wrong with the GPU, maybe damage from mining :S

Back to normal.
I just waited an hour and tried again.

I must have some third party
extensions causing this.

Update:
You were right.
The intensity was too high.
Set it back one and now there are 0 HW errors.
My hash rate is off by 50% though.
